How do I access my IN Focus First patient portal?
Click the “Patient Portal Login” button above to log into your Charm Health patient portal. You can also download the Charm IN Focus First app on iPhone or Android for easy mobile access to your records, messages, and appointments.
How do I update my credit card on file at IN Focus First?
Click “Update Your Card on File” above. You will be directed to our secure Acuity Scheduling page where you can log in or create an account to update your payment information.
How do I request a medical records release?
Click the “Indiana Medical Release of Information Form” button above to download the form. Complete it and return it to our office by email or fax.
How do I reschedule or cancel my appointment?
You can reschedule or cancel your appointment through your appointment email confirmation or by contacting our office directly up to 48 hours prior to your appointment date and time. Please review our cancellation policy to avoid late cancellation fees.
How do I message my provider?
Log into your Charm patient portal and use the secure messaging feature to contact your psychiatric provider. This is the fastest and most secure way to reach your care team.
What do I do if I’m experiencing a mental health crisis?
If you are experiencing a psychiatric emergency, call 988 (Suicide & Crisis Lifeline) or go to your nearest emergency room. For non-emergency concerns, message your provider through the Charm patient portal.
Does IN Focus First accept insurance?
IN Focus First is a private pay psychiatric practice. We accept HSA, FSA, Google Pay, and all major credit cards. We provide superbill documentation upon request that you can submit to your insurance for possible out-of-network reimbursement. Contact your insurance for details.
